1、开办第二、三类医疗器体经营企业，应向经营所在地食品药品监管分局申请并提交以下资料
(1)、《北京市医疗器材经营企业许可证申请表》;
(2)、《医疗器材经营企业许可证申请材料登记表》;
(3)、工商行改管理部门出具的企业名称预技准证明文件成《营业执照》气印件(校验口件);
(4)、拟办企业质量管理负责人的身份证，学历我者职你证明复印件及个人简历
(5)、拟办企业委量管理人员的身份证、学历成者职称证明气印件
(6)、拟办企业组织机构与职能或专职质量管理人员的职
